The Shin Sumo rikishi wear a leotard over which is a representation of
the mawashi.  

In historic times, there was women's "sumo" where all that was worn was
a mawashi.  Obviously, this was not sumo, but rather an historic
equivalent of mud wrestling - an entertainment set up for the prurient
interests of the paying customers.  The "shikona" of the women were
often ribald .  Other such entertainments included one-man sumo (often
done satirically where a single entertainer protrayed both rikishi in a
bout and let the crowds cheers decide who was the winner) or matching
one of the women against a blind man.
