Hoshi entry difficulty

For some reason, I am not able to get the Hoshi web page to take my entry.

I am using Netscape 3.0, the same shikona and password as on the prior
several occasions, and tried it twice to be sure I had not entered anything
other than requested information (no spaces, etc.).

I also requested the FAQ sent so I could just email it, but that also did
not come through, only the pre-message announcing it would be sent separately.  

So, I am stuck!  Is anyone else having this problem?  Is it me?  Could
someone send me a copy of the hoshi entry form for Aki?  I would appreciate it.
