This is my understanding without consulting any written materials.

You should remember the last application of Koushou. I think it was
Wakanohana. He injured and it was ruled "Koushou". He stay away from
matches rest of the basho and all next basho. After losing record of
injured basho and skipping the next basho, when he came back, he was
"kado-ban" ozeki. He needed to have a winning record to stay in Ozeki rank.
That means that the record of basho he got injured was recorded as
makekoshi, but the basho he skipped totally for his injury was not recorded
as makekoshi.

If this example was applied to the case of Musoyama and Kotonishiki, they
will lose their Sanyaku position for Haru-basho. If they need to skip
Harubasho for treatment and rehabilitation, they don't need to worry about
further dropping in banzuke for Natsu-basho.
